Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. If you have any concerns please discuss these with your doctor or receptionist staff.

Further information regarding the practice is available from reception. If you have any problems we would like to hear about them. Please discuss with a Doctor or Receptionist staff. A "Comment Box" is located in a private area of the clinic for productive suggestions which may improve quality of care. If you feel your problems cannot be resolved, contact Health Service Commissioner on 1800 136 066.

This clinic has a NO SMOKING POLICY.
